Prairie Heart Institute (PHI) is the largest cardiac center in Illinois. PHI identified several challenges in its transition to value based care, including variability in protocols, gaps in patient education materials, lack of real-time monitoring for patient compliance, and increased cost due to higher than expected length of stay, readmissions and discharge to skilled nursing facilities (SNF).
To address these challenges, PHI partnered with 草莓社区 and achieved the following results:
- 鈫 1.0 days length of stay (LOS)
- 鈫 45% 30-day readmissions
- 鈫 64% discharged to Skilled Nursing Facilities
- Improved patient experience across cardiac surgery and cardiac electrophysiology
